Abstract
Scleral Buckling (SB) is an established technique for treatment of Rhegmatogenous Retinal Detachment with a final success rate of 95 %. Yet, there has been a decline in the training and practice of SB. More surgical and healing time may be the cause. We present modified SB technique for simple retinal detachment where Segmental SB with drainage is done using 276 silicone tire.
Procedure
After conjunctival peritomy of one quadrant, two recti muscles are hooked and isolated. The break is identified and cryopexy done. 276 tire is sized and placed below the recti to cover the break completely. Using 4-0 ethibond, mattress sutures 6 mm long and of half sclera depth are placed. Subretinal fluid drainage is done and air injection given. After indirect ophthalmoscopy, the final tie is done, conjunctiva closed.
Advantages
Attached retina with reduced surgical time, quiet eye on post-op day 1, minimal refractive error, early restoration of vision, non-cataractogenous procedure.
