Abstract

A 52-year old male presented with poor vision in right eye. He gave history of two refractive surgeries in RE many years ago & history of cataract surgery in both eyes 13 years back. No surgical records were available. On examination, RE showed radial corneal scars suggestive of past radial keratotomy, with aphakia. LE showed PCIOL in situ. Dilatation revealed myopic fundus with PPA. RE showed no subluxated IOL, but the patient told that a lens was implanted. Hence, the patient was posted for PPV with SFIOL implantation.

Intra-operatively, an ICL- implantable collamer lens was noted in the vitreous cavity. It was explanted & sutured SFIOL was implanted. How the ICL landed up in the vitreous cavity in a pseudophakic eye, remains a mystery!

Post-operatively, the patient did well.

Conclusion: Every vitrectomy surgery is a surprise- more so in cases where reliable history & previous surgical records are not available. A surgeon should be prepared to handle such un-anticipated instances.
