Abstract
Cataract surgery is one of the most commonly performed procedures and phacoemulsification has made it more predictable and excellent visual outcomes. Complication rates have drastically reduced due to good visualization, instrumentation and surgical expertise. However surgeons sometimes face a dreaded complication like a nucleus drop during a routine cataract surgery. This requires a management by posterior segment surgeon wherein the nucleus is emulsified in the vitreous cavity itself using a fragmatome or taken out through a scleral tunnel. A fragmatome is a specialized instrument specific in the management of nucleus drop and involves significant investment. Here we report the use of phaco handpiece used as a fragmatome in the management of nucleus drop in two cases. The aim of this video is to demonstrate a simple and effective method to convert a phaco handpiece to a fragmatome making it economical considering its rare use.
