Abstract
Straatsma syndrome is a triad of unilateral extensive myelinated nerve fibre with high myopia and amblyopia. We report two young male patients having unilateral Straatsma Syndrome with exotropia and other eye being absolutely normal. Both the patients were subjected to amblyopia therapy but the prognosis was guarded in this disease. Their profound visual impairment, exotropia, and the early age onset of symptoms indicated that the amblyopia may have been organically caused delay in diagnosis, the presence of intense amblyopia, and superimposed strabismus increase the likelihood that amblyopia will be partially or totally resistant to therapy. Extraocular muscle surgery can also be performed in patient requiring cosmetic correction
