Abstract
An 82-year-old female with a history of infiltrating ductal cell carcinoma of the breast [ER+, PR+, Her Neu2 negative] treated with MRM, chemotherapy and radiation three years ago currently on Tamoxifen 20 mg daily, presented with diminished vision in left eye for 2 weeks. OS BCVA was 6/24P. A 3DD by 3DD solitary circumscribed elevated lesion with surface pigment mottling and subretinal fluid at the edge was seen. OCT revealed multiple SRF pockets with speckles, shaggy photoreceptors and a lumpy bumpy elevated choroidal lesion. SW-FAF, FFA and ICGA findings were consistent with choroidal metastasis.
The oncologist initiated palliative chemotherapy with CDK inhibitor Palbociclib and Letrozole, a non steroidal aromatase inhibitor. At the latest visit, BCVA was 6/9 OS with complete regression of the choroidal lesion and SRF.
To our knowledge, this is the first case to report regression of choroidal metastasis with Palbociclib in a primary malignancy resistant to selective ER modulator.
