Abstract
A 34-year-old lady came with complaint of sudden diminished vision in both the eyes since 7 days.Her best-corrected visual acuity (BCVA) was 6/18 in the right eye and 6/36 in the left eye.No anterior chamber inflammation or vitritis was seen.She presented with subtle multiple yellowish-white lesions in the posterior pole,bilaterally. General examination and systemic investigations were unremarkable. SD-OCT had multiple pockets of serous detachment, suggestive of Vogt koyanagi Harada syndrome (VKH).But the subtle placoid lesions were accentuated by OCTA (OCT angiography) and later fluorescein angiography,diagnostic of acute posterior multifocal placoid pigment epitheliopathy. As the patient had decreased BCVA with centromacular disease involvement, treatment with oral prednisolone was started after ruling out infectious causes.After 2 weeks,her BCVA improved to 6/9 in both the eyes.Thus,a non invasive test OCTA helped us in clinching the diagnosis in absence of typical fundus findings.
