This surgical video is constellation of three challenging scenarios in patients of retinitis pigmentosa. 1st case is of a 75 year old patient with macular hole with foveo-schisis detachment with extensive pigmentary changes and chorioretinal atrophy. 2nd patient of 45 years age had macular RPE hyperplasia with foveoschisis. 3rd patient of 26 years age had macular RPE hyperplasia with pseudo – nuclear fragment like asteroid lesions at posterior pole. Patients underwent modified vitrectomies to close the hole, flatten the foveoschisis and stabilize the posterior pole anatomy. Discerning hole and ILM from surrounding scarring was difficult in the milky – way of first case. Second case posed a challenge of operating in the macular black – hole. Third case had nearly committed the surgeon to blunder in destroying masquerading asteroid – like lesions with catastrophic consequences. Postoperatively patient’s retinal galaxy was saved with stable vision.
