Abstract
A case of bilateral exudative RD,suspected of VKH treated with steroids was referred to us due to non response to treatment.Further evaluation was suggestive of bilateral multifocal CSCR and treated with focal laser.OD responded to focal laser,but exudative RD increased in OS.Quadrantic sclerectomies with vortex vein decompression was done.Scleral windows of 4-5*4-5mm made in all 4 quadrants by lamellar dissection of sclera.Sclera was noted to be thick during dissection.Full thickness sclerostomies are made in windows by scraping scleral bed with 15degree sideport blade until choroidal hue and egression of suprachoroidal fluid was seen.The bed is cauterized to keep sclerostomy open.In inferior two quadrants posterior border of sclera windows extended upto vortex vein to do vortex vein decompression and trans scleral external drainage of SRF perfomed with simultaneous injection of normal saline.Near total drainage was performed.Retina was attached on first post operative day
