Abstract
Methods: While operating on a 65-year-old female cataract patient with a mid-dilated pupil, resistance was felt during CCC with a simultaneous appearance of radial stress lines indicating more anteriorly attached zonular fibers. Rebounding of free flap near the tearing margin was noted and failure to make the free flap flat on the anterior capsule helped in detecting this anatomical anomaly. Zonular fibers are severed meticulously whenever they are encountered during CCC with a 26 gauge needle (bevel) resulting in a somewhat circular capsulorhexis. Surgery was completed with a rigid in-the-bag intraocular lens implantation.
Conclusion: Identification of this rare aberration is important to prevent rhexis-running during CCC although achieving a circular rhexis is tedious. It is for surgeons to think about the possibilities of this anatomical anomaly to prevent surgical complications.
